ABSTRACT

Patients and methods: This cross-sectional survey took place in May, 2018 in Kodjokro, a village in the south-east of Côte d'Ivoire. The study population consisted of women of reproductive age, from 15 to 49 years old. A questionnaire was used to collect data on socio-demographic, gyneco-obstetrical characteristics, knowledge and attitudes and he use of contraceptive methods. The chi-square test was used to measure the associations between the use of contraceptive methods and each of the characteristics studied

ABSTRACT

Objective: Early detection and effective management of high-risk pregnancies can substantially contribute to the reduction of adverse maternal and fetal outcomes. This study aimed to determine the prevalence and clinical profile of women with high-risk pregnancies in rural areas who utilize antenatal services in a primary health center (PHC).Materials and Methods: A retrospective analysis was carried out over a six-month period by reviewing the mother and child protection cards maintained at the PHC’s Maternal and Child Health Center. During the study period, 950 pregnant women were registered, of whom 793 were included in the study based on the completeness of the records. Data analysis was performed using the licensed Statistical Package for the Social Sciences (SPSS) software version 21.0.Results: The prevalence of high-risk pregnancy among the antenatal women was 272 (34.3%) with 95% CI [31.1–37.7]. Of the 272 women, 240 (88.2%) had a single high-risk factor, while 32 (11.8%) had more than one high-risk factor. The major factor contributing to high-risk pregnancy was hypothyroidism (43.7% with 95% CI [37.9–49.6]), followed by a previous lower segment Caesarean section (LSCS) (19.1%).Conclusion: The study found that the prevalence of high-risk pregnancies was 34.3% in this rural setting. The majority of high-risk pregnancies were due to hypothyroidism, followed by more than one previous LSCS or abortion. Further research is required to track high-risk pregnancy outcomes and investigate the newborn thyroid profile of women with hypothyroidism.

ABSTRACT

@#Introduction: This study aims to provide the current situation of leprosy prevalence and clinical findings in one of the endemic regions of North Gresik City that has already achieved elimination status. Leprosy remains a public health problem in some developing countries, including Indonesia. Indonesia, with two other big countries, India and Brazil, contributed 74% of all the new leprosy cases detected. Even though Indonesia achieved the elimination target from WHO in 2018, the endemic pockets are still scattered, and new cases remain detected each year. Methods: This study is a one-decade retrospective analysis descriptive record based on newly diagnosed cases of leprosy from 2010 until 2020. We use total data sampling of all newly diagnosed leprosy cases and analyze using SPSS. Result: This study found that newly diagnosed cases of leprosy are still discovered each year. 15.7% of newly diagnosed cases were children. MB type was dominated type in this study. MB types are statistically significant in male subjects (35.5%) diagnosed more than one year after the symptoms appear. 40.7% were found with deformity when first diagnosed. Conclusion: This study reports a high rate of contracting leprosy disease in children, with 15.7% of cases. It indicates that disease transmission remains high in the community and might be out of control. The new cases diagnosed dominated with MB type, and 40.7% had deformity when first diagnosed. There was a delayed diagnosis reported that could be caused by ineffective surveillance for early diagnosing new cases.

ABSTRACT

Objective@#To investigate the current status and influencing factors of knowledge, attitude, and practice of hypertension prevention and control among rural residents in Shanxi Province, so as to provide insights into implementation of health education pertaining to hypertension control in rural areas. @*Methods@#A total of 7 403 residents were sampled using a multi-stage cluster random sampling method from rural areas of Yangqu, Daning, and Yonghe counties of Shanxi Province from November 2020 to July 2021 for face-to-face questionnaire surveys and physical examinations. Residents' basic characteristics, knowledge, attitude, and practice of hypertension prevention and control, and height, weight, blood pressure were collected, and factors affecting knowledge, attitude, and practice of hypertension prevention and control were identified using a generalized linear model. @*Results@#A total of 7 403 questionnaires were allocated, and 7 031 effective questionnaires were recovered, with an effective response rate of 94.98%. The respondents included 3 035 men (43.17%) and 3 996 women (56.83%), and had a mean age of (58.45±13.75) years. The median scores of hypertension prevention and control knowledge, attitude and practice were 5 (interquartile range, 4), 16 (interquartile range, 5) and 14 (interquartile range, 7) points, with scoring percentages of 48.40%, 80.55% and 54.41%, respectively. Generalized linear model analysis showed that an educational level of junior high school and above, annual family income per capita of ≥4 000 RMB, family history of hypertension, development of hypertension among family members or friends, overweight/obesity and presence of depression resulted in high scores for hypertension prevention and control knowledge; annual family income per capita of ≥10 000 RMB, family history of hypertension, presence of hypertension among family members or friends, mild/severe insomnia and presence of depression resulted in high scores for hypertension prevention and control attitude, while age of ≥65 years and blood pressure of ≥140/≥90 mmHg resulted in low scores for attitude; females, age of ≥45 years, an educational level of junior high school and above, blood pressure of ≥140/≥90 mmHg, family history of hypertension, presence of hypertension among family members or friends and overweight/obesity resulted in high scores for hypertension prevention and control practice, and residents had a low body weight had a low score for practice (all P<0.05).@*Conclusions@#The hypertension prevention and control attitude is satisfactory among rural residents in Shanxi Province; however, the hypertension prevention and control knowledge and practice are poor. Health education pertaining to hypertension prevention and control should be strengthened, and personalized health education is needed targeting individuals with a low educational level and low income.

ABSTRACT

Objective@#To investigate the influencing factors of depressive symptoms among rural elderly patients with chronic diseases in China, so as to provide insights into depression prevention and control among the rural elderly patients with chronic diseases.@*Methods@#The basic demographics, health status, and lifestyle of rural residents at ages of 65 years and older who had at least one chronic disease were retrieved from The Chinese Longitudinal Healthy Longevity Survey (CLHLS) database in 2018, and participants' depressive symptoms were assessed with The Center for Epidemiological Studies Depression-10 (CES-D-10) scale. Factors affecting the depressive symptoms were identified with a multivariable logistic regression model. @*Results@#Totally 5 146 rural elderly patients with chronic diseases were enrolled, including 2 373 men (46.11%) and 2 773 women (53.89%). The prevalence of depressive symptoms was 27.13%. Multivariable logistic regression analysis identified having two and more children (OR=0.614, 95%CI: 0.387-0.975), living alone (OR=1.450, 95%CI: 1.192-1.764), life satisfaction (general, OR=1.933, 95%CI: 1.651-2.264; low, OR=5.366, 95%CI: 3.488-8.254), self-assessed health status (general, OR=2.697, 95%CI: 2.284-3.185; poor, OR=5.338, 95%CI: 4.262-6.685), disability in instrumental activities of daily living (OR=1.592, 95%CI: 1.328-1.908), sleep duration (normal, OR=0.502, 95%CI: 0.429-0.586; too long, OR=0.494, 95%CI: 0.405-0.603), exercise (OR=0.721, 95%CI: 0.607-0.856), watching TV (OR=0.787, 95%CI: 0.664-0.933), and gardening activities (OR=0.781, 95%CI: 0.626-0.975) as factors affecting depressive symptoms among rural elderly patients with chronic diseases.@*Conclusions@#The prevalence of depressive symptoms was 27.13% among rural elderly patients with chronic diseases. Number of children, living style, life satisfaction, health status, sleep duration, exercise, watching TV, and gardening activities are associated with the development of depressive symptoms among rural elderly patients with chronic diseases.

ABSTRACT

Objective: To assess and compare the knowledge, attitude, and practice of mothers of pre-school children in urban and rural areas toward maintaining child oral health.Materials and Methods: A cross-sectional comparative study was conducted in a convenient sample of 400 mothers of pre-school children (aged 9 months to 3 years) visiting the outpatient department of the Department of Dentistry, Shyam Shah Medical College, Rewa (M.P), India between February 1 and April 21, 2022. The study sample was divided into two groups based on whether they belonged to a rural area (Group A) or urban area (Group B). Group A included 236 participants, and Group B 186 participants. They were served a 24-item questionnaire to assess and compare their knowledge, attitude, and practice toward child oral health. The data were represented as mean and proportions, and a student t test and analysis of variance were performed.Results: In Group A and Group B, 93.64% and 98.17% mothers respectively had the knowledge of cariogenic food. While 66.10% of mothers in Group A and 73.78% in Group B were aware that regular cleaning of teeth protects against tooth decay, only 5.49% of mothers in Group B and none in Group A reported taking their children to routine dental visits; the majority (94.07% in Group A and 78.05% in Group B) had never taken their children to the dentist. Urban background was significantly associated with better knowledge, attitude, and practice scores. Mother’s’ age and education level were significantly associated with better Knowledge, Attitude, and Practice scores in both groups.Conclusion: Mothers can play an important role in promoting positive attitudes and healthy practices toward oral health care in children. Knowledge, attitude, and practice score were found to be significantly low in mothers who belonged to the rural subgroup.

ABSTRACT

Objective@#To investigate the influencing factors of infectious disease-specific health literacy (IDSHL) among rural residents in Dongxiang Autonomous County, and to construct a nomogram-based model for prediction of IDSHL.@*Methods@#Totally 1 250 rural residents at ages of 15 years and older were sampled from Dongxiang Autonomous County using a stratified random sampling method. Participants' IDSHL was evaluated using the IDSHL Assessment Scale among Chinese Residents, and factors affecting the participants' IDSHL were identified using a multivariable logistic regression model. A nomogram-based model was created, and the predictive effectiveness of this model was evaluated using the receiver operating characteristic (ROC) curve, Hosmer-Lemeshow test and C-index.@*Results@#A total of 1 223 valid respondents were enrolled, including 687 men (56.17%) and 536 women (43.83%), and the proportion of IDSHL was 48.48%. Multivariable logistic regression analysis identified age (reference: 60 years and older; 30 to <40 years: OR=4.273, 95%CI: 2.397-7.617; 40 to <50 years: OR=3.938, 95%CI: 2.238-6.928), education level (reference: illiteracy/semi-illiteracy; primary school: OR=2.140, 95%CI: 1.456-3.144; high school/vocational high school/technical secondary school: OR=2.914, 95%CI: 1.652-5.138; junior college and above: OR=4.514, 95%CI: 2.261-9.011), healthcare seeking/medications in the past 2 weeks (reference: yes; no: OR=2.025, 95%CI: 1.346-3.046), self-rated health (reference: good; generally: OR=0.603, 95%CI: 0.376-0.966; poor: OR=0.462, 95%CI: 0.284-0.751) and daily average duration spent online (reference: no internet access; <1 h: OR=1.859, 95%CI: 1.306-3.437; 1 to <2 h, OR=1.996, 95%CI: 1.344-3.380; 2 to <3 h: OR=2.132, 95%CI: 1.109-3.116; 3 h and longer: OR=2.119, 95%CI: 1.175-3.390) as factors affecting IDSHL among rural residents in Dongxiang Autonomous County. The area under the ROC curve of the model was 0.774 (95%CI: 0.741-0.807) and the model had high calibration and differentiation levels [Hosmer-Lemeshow test: χ2=13.276, P=0.103; internal model validation (bootstrapping): mean absolute error=0.019; C-index=0.764].@*Conclusions@#Age, education level, healthcare seeking/medications in the past 2 weeks, self-rated health status and daily average duration spent online are factors affecting IDSHL among rural residents in Dongxiang Autonomous County. The nomogram model created based on these factors has a high efficiency and applicability for prediction of IDSHL among rural residents in Dongxiang Autonomous County.

ABSTRACT

Introduction: In developed countries, about 50% of all Low-Birth-Weight new born are preterm. Low birth weight is also a leading cause of neonatal death and major risk factor for infant and under-five morbidity and mortality. Objectives: To find out prevalence of low birth weight& to determine the association of socio-demographic variables, maternal medical and obstetric risk factors with low birth weight. Methodology: Across sectional community-based study was conducted in a rural field practice area of a medical college. A total of 600 participants (mothers who delivered within 3 months) were included in the study. Results: The prevalence of LBW was found to be 17% in the current study. Statistically significant asso-ciation was obtained between age, education, occupation, socio-economic class of participants and birth weight of the child. The prevalence of LBW was found to be significantly high (51.7%) amongst children of those participants who consumed/used tobacco in any form. Conclusion: The prevalence of the LBW was found to be 17%. Extremes of reproductive age group, illit-eracy, occupation of labourer, SEC class IV, grand-multiparity, inadequate IFA consumption, short stat-ure, weight of mother and use of tobacco were found to be significantly affecting the birth weight and as-sociated with low birth weight.

ABSTRACT

INTRODUCCIÓN: las enfermedades parasitarias representan un problema de salud pública debido a su alta prevalencia por todo el mundo sobre todo en países en desarrollo, especialmente en áreas rurales y Bolivia no es la excepción. OBJETIVO: el presente estudio tuvo el objetivo de determinar la frecuencia y distribución de enteroparásitos en 8 municipios rurales del departamento de La Paz durante el periodo de agosto a septiembre de 2014. MATERIALES Y MÉTODOS: el trabajo es un estudio tipo transversal-descriptivo con un universo de trabajo de 1238 muestras de heces fecales conservadas con formol, las cuales fueron enviadas por el equipo médico de SUYANA (organización no gubernamental sin fines de lucro) a los laboratorios del Instituto SELADIS (Servicios de Laboratorio de Diagnóstico e Investigación en Salud). RESULTADOS: se realizaron estudios coproparasitológicos directos de cada una de las muestras y la observación microscópica dio los siguientes resultados. Se evidencio la presencia de enteroparásitos en 89,5% de la población estudiada, de los cuales 97,2% representan protozoarios tales como B. hominis, E. coli (protoozoos comensales) y G. lamblia (protozoo intestinal patógeno) los cuales estarían como los de mayor distribución, por otro lado el 2.8% de la población total corresponden a helmintos donde H. nana tiene una distribución de 1,8%, A. lumbricoides 0,7% y T. trichiura, S. stercoralis, Uncinarias estarían en el 0,1% de la población. También se pudo evidenciar que la mayor distribución de enteroparásitos estaría entre 1-10 años de edad (37,4%). Finalmente se evidencio que de las 8 poblaciones de estudio Charazani, Calacoto, Comanche presentarían mayor distribución de enteroparásitos (10-11 parásitos). CONCLUSIÓN: se evidencio que casi el 90% de la población en estudio, presentarían parásitos intestinales, con un claro predominio de los protozoarios sobre los helmintos. Esta información epidemiológica servirá de apoyo para mejorar los programas de salud en estas poblaciones.


INTRODUCTION: parasitic diseases represent a public health problem because of its high prevalence throughout the world, especially in developing countries, particularly in rural areas and Bolivia is no exception. OBJETIVE: therefore, the present study had the objective of determining the frequency and distribution of enteroparasites in eight municipalities of La Paz between August and September in 2014. MATERIALS AND METHODS: this was a cross-sectional-descriptive study with a universe of 1238 formalin-preserved stool samples, which were sent for analysis by the medical team of SUYANA (a non-profi t organization) to the Bolivian Institute of Health Diagnostic and Research Laboratory Services (SELADIS, acronym in Spanish). RESULTS: Copro-parasitological studies of each sample and microscopic analysis were detected. As a result, prevalence of infection by any given enteroparasites were detected, 89.5% of protozoans (B. hominis, E. coli (commensal protozoa) and 97.2%, G. lamblia (pathogenic intestinal protozoa) which represent the highest distribution. Besides, we identified, 2.8% of helmintos, among them, 1.8% of H. nana, 1.7% of A. lumbricoides and 0.1% of others (T. trichiura, S. stercoralis and Uncinarias). According to age group, 37.4% were people between 1 and 10 years old (enteroparasites infection). From eight municipalities, Charazani, Calacoto, and Comanche had the highest distribution of enteparasites (10-11 parasites). CONCLUSIONS: the present study showed that 90% of the population had intestinal parasites, where protozoans were higher than helminths. This epidemiological information could be reliable to improve health care programs for these populations.

ABSTRACT

Background: Adolescent is the period of the life between the ages of 10-19 years. Adolescence is time to learn and adopt healthy habits to avoid many health and nutritional problems later in life. Objective of this study is to assess the nutritional status of adolescent girls by anthropometric measurements and to determine the association between different individual and family level factors with under nutrition. Methods: It is a community based cross-sectional study conducted among adolescent girls of age between 10-19 years in rural areas of Tirupati. The study sample was 188. BMI for age <-2SD is considered as thinness. A pre tested semi structured questionnaire was used to collect individual and family level factors and analysed using appropriate statistical tests with SPSS26th version. Results: Prevalence of thinness and overweight/obese among adolescent girls in this study was 17% and 17.6%. Thinness was significantly high among girls with family size is more than five(23.8% vs 8.4%), who attained menarche at >11 years (17.1% vs 0%) and who are not doing regular physical activity(22.3% vs 9.2%). Multivariate binary logistic regression showed socioeconomic status, family size and physical activity are significant predictors of thinness. Conclusions: It can be concluded from this study that thinness was significantly high among adolescent girls with family size >5, who attained menarche at >11 years and who does not have the habit of doing regular physical activity.

Objective@#To evaluate the caries preventive effect and economic outcome of an oral health promotion model in rural primary schools in Guangxi Zhuang Autonomous Region.@*Methods@# In November 2015, a baseline survey was conducted on the caries prevalence of rural first graders in program areas, and then a program intervention was carried out on the first graders of target primary schools. The intervention methods included the distribution of oral health care products, oral health education, teacher training, etc. In 2020, the method of cluster random sampling was adopted to include the sixth graders who received the program intervention as first graders and were still available in 2020 as the intervention group. During this period, the sixth graders transferred from foreign schools who failed to receive the program intervention as first graders were included in the non-intervention group. The caries preventive effect was evaluated by analyzing the caries epidemiological data of 2 652 children aged 11-13 years, comparing the intervention and non-intervention group. The cost of the program was estimated by the structural analysis method. A decision tree model was established by TreeAge pro2019 and incorporated into the effect parameters and cost parameters for cost-effectiveness analysis and sensitivity analysis.@* Results @# The prevalence of caries in the intervention group was 54.8%, and the mean DMFT was 1.36 ± 1.64, both of which were lower than those in the non-intervention group. The difference in prevalence and mean DMFT between the two groups was statistically significant (P < 0.05). It costs CNY 319.83 per child to reduce suffering from caries. The number of patients with caries in the intervention group was the most sensitive indicator of an economic effect. The probability of a cost-effectiveness advantage for the program was 92.2%. @*Conclusion @# This oral health promotion model used in rural schools demonstrated a caries preventive effect that was very likely economically advantageous. It is of practical significance to improve and promote this model in the Guangxi Zhuang Autonomous Region.

ABSTRACT

Objective To understand the growth and dietary diversity status of children in multi-ethnic areas of Sichuan province,and to explore the associations of dietary diversity with growth and development indicators. Methods Children of 18-36 months old and their primary caregivers were selected with multi-stage cluster random sampling method from rural areas of Han,Tibetan,and Yi ethnic groups in Sichuan province. The sociodemographic information of children and their caregivers was collected using self-designed questionnaire.The dietary diversity score(DDS)was calculated according to the criteria in the Guidelines for Measuring Household and Individual Dietary Diversity released by the Food and Agriculture Organization of the United Nations.The body height(length)and body weight of each child were measured by standard equipment for anthropological measurement,and the height for age Z score(HAZ),weight for age Z score(WAZ),and weight for height Z score(WHZ) were calculated.Multivariate linear regression was performed to analyze the relationship between dietary diversity and growth indicators of children. Results A total of 1092 children were enrolled in this study,and the prevalence of stunting(HAZ<-2),underweight(WAZ<-2),and wasting(WHZ<-2)was 21.1%,4.9%,and 2.5%,respectively.The children had the mean DDS of 4.8±1.7,and 45.3% of children had poor dietary diversity(DDS≤4).The children of Han ethnic group(5.8±1.4)had higher DDS than those of Tibetan ethnic group(4.9±1.6)and Yi ethnic group(3.9±1.6)(P<0.001).The results of multivariate linear regression indicated that DDS was positively correlated with HAZ(β=0.206,95%CI=0.158-0.254,P<0.001)after adjustment of sex,age,birth weight,preterm birth,and parental body height.After further adjustment of family fixed assets,ethnic group,caregiver's type,and caregiver's education background,the correlation between DDS and HAZ remained significant(β=0.077,95%CI=0.026-0.128, P=0.003). Conclusions The children in the multi-ethnic rural areas of Sichuan province showed troublesome growth and development status and low dietary diversity,which were conspicuously different between ethnic groups,especially in the rural areas of Yi ethnic group.The dietary diversity was positively associated with HAZ.It is recommended to carry out nutrition and health education according to the local dietary characteristics and thus improve the growth and development of children in multi-ethnic rural areas in Sichuan.

ABSTRACT

Objective We used standardized patients to evaluate the accuracy and explore the influencing factors of the diagnosis of unstable angina pectoris and type 2 diabetes by primary healthcare providers in Sichuan rural areas,aiming to provide a scientific basis for improving the diagnosis accuracy of primary healthcare providers for the two chronic diseases. Methods A multi-stage stratified random cluster sampling method was adopted to select 100 villages from 50 townships in 5 districts/counties in Zigong city,Sichuan province. General and internal medicine practioners who were on duty on the survey day were enrolled in the survey.Two rounds of data collection were conducted.In the first round,the basic information of providers from township health centers and village clinics was collected.One month after the the first survey,standardized patients were used to collect the information related to the diagnosis of unstable angina pectoris and type 2 diabetes by rural primary providers.Logistic regression was carried out to analyze the factors influencing the diagnosis accuracy. Results A total of 172 rural primary healthcare providers were enrolled in the survey,who completed 186 standardized patient visits and showed the correct diagnosis rate of 48.39%.Specifically,the correct diagnosis rates of unstable angina pectoris and type 2 diabetes were 18.68%(17/91) and 76.84%(73/95),respectively.The providers with medical practitioner qualifications were more likely to make correct diagnosis(OR=4.857,95%CI=1.076-21.933, P=0.040).The providers who involved more necessary consultation and examination items in the diagnosis process had higher probability of correct diagnosis(OR=1.627,95%CI=1.065-2.485, P=0.024).Additionally,the providers were more likely to make a correct diagnosis for type 2 diabetes than for unstable angina pectoris(OR=6.306,95%CI=3.611-11.013, P<0.001). Conclusions The overall diagnosis accuracy of unstable angina pectoris and type 2 diabetes was relatively low among primary healthcare providers in Sichuan rural areas.The training of diagnosis process can be taken as a key for improving providers' practice ability so as to increase the diagnosis accuracy of chronic diseases.

ABSTRACT

Background Indoor air pollution is an important risk factor affecting health of the respiratory system. Studies on indoor air pollution in China are mostly limited to the central and eastern regions, and there are few studies in the rural areas of northwest China. Objective To explore the influencing factors of lung ventilation function and its relationship with indoor air pollution in rural areas of Gansu Province based on a cross-sectional investigation. Methods A total of 399 subjects were selected from four villages in Baiyin and Yuzhong of Gansu Province. Questionnaires were used to collect demographic information, lifestyle, disease history, fuel use, and other information, and physical and functional tests were ordered such as height, weight, and lung function. The Indoor Air Pollution (IAP) exposure index was calculated based on smoking, fuel type, and weekly ventilation. IAP > 5 was defined as a high level of indoor air pollution. Lung function indexes included forced vital capacity (FVC), forced expiratory volume in the first second (FEV1), FEV1/FVC, forced vital capacity as a percentage of predicted value (FVC%), and forced expiratory volume in the first second as a percentage of predicted value (FEV1%), which were used to determine pulmonary ventilation dysfunction. Logistic regression model was used to evaluate the relationship between indoor air pollution and pulmonary ventilation function, and subgroup analysis was further conducted according to home address and BMI , in order to identify the high-risk population of pulmonary ventilation dysfunction. Results The mean age of the subjects was (56.75±7.31) years old; 155 subjects (38.85%) had normal pulmonary ventilation function, and the other 244 subjects (61.14%) had impaired pulmonary ventilation function; about 89.97% of the subjects were exposed to high level of indoor air pollution (IAP > 5). We found that IAP > 5 (OR=2.327, 95%CI: 1.089-4.974) and use of bituminous coal as the main heating fuel in winter (OR=3.467, 95%CI: 1.197-10.037) increased the risk of pulmonary ventilation dysfunction after adjusting for age, BMI, residence, gender, smoking, drinking, and cardiovascular disease. The subgroup analysis results showed that no ventilation in the living room/bedroom (OR=3.460, 95%CI: 1.116-10.268) increased the risk of pulmonary ventilation dysfunction in Baiyin. Heating with coal stoves and Chinese Kang in the bedroom (OR=2.092, 95%CI: 1.030-4.247) and cooking in the bedroom in winter ( OR =2.954, 95% CI : 1.046-8.344) also increased the risk of pulmonary ventilation dysfunction in the residents with BMI≤24 kg·m−2. IAP > 5 (OR=3.739, 95%CI: 1.147-12.182) was associated with a significantly increased risk of pulmonary ventilation dysfunction in the BMI > 24 kg·m−2 subgroup. Conclusion The pulmonary ventilation function of rural residents in Gansu is poor, which is negatively correlated with indoor air pollution. Coal use, overweight, cooking in bedroom, and use of coal stoves and Chinese Kang for heating may increase the risk of pulmonary ventilation dysfunction, while room ventilation is a beneficial factor.

O presente estudo teve como objetivo identificar e comparar os níveis motivacionais para a prática esportiva de escolares de instituições rurais e urbanas. Ao todo participaram do estudo 848 estudantes, sendo 424 da escola do campo e 424 da escola urbana, com idades compreendidas entre 10 a 19 anos de ambos os sexos. Para a avaliação do quadro motivacional esportivo foi utilizado o instrumento de pesquisa "Sport Motivation Scale" (SMS), que visa mensurar e avaliar componentes intrínsecos e extrínsecos da motivação. Para a análise estatística foram utilizados os testes de Mann Whitney para comparação de grupos e o teste qui quadrado para a associação das variáveis. Após a interpretação dos dados, os resultados indicaram que os alunos das zonas rurais de ensino encontravam-se mais motivados que os estudantes do perímetro urbano, sobretudo, quando a comparação se deu entre escolares do sexo feminino e em etapas mais avançadas de escolaridade. Em consonância a esses dados, quando realizado um teste de associação entre o tipo de escola e a classificação motivacional, constatou-se que a maioria dos discentes das escolas rurais estavam classificados em um nível de motivação considerado alto. Em suma, a partir deste estudo, verificamos que o ambiente onde os escolares estudam ou residem, em alguma medida, pode influenciar na motivação esportiva. Embora não esteja claro as razões pelas quais os escolares do campo se evidenciaram mais motivados, acreditamos que no contexto rural existe uma limitação quanto à diversidade de atividades, o que acaba contribuindo para uma maior atenção para as práticas esportivas. Como na cidade o jovem tem a sua disposição um leque maior de espaços de convivência social, é provável que o esporte passe a ser dividido com outras ocupações e, por isso, ocorra um maior arrefecimento motivacional no espaço urbano quando comparado com o meio rural.(AU)


The present study aimed to identify and compare the motivational levels for the sport practice of students from rural and urban schools. The study included 848 students, being 424 of the rural school and 424 of the urban school, aged between 10 and 19 years of both sexes. For the evaluation of the sport motivational framework, we used the research instrument "Sport Motivation Scale" (SMS) to measure and evaluate the intrinsic and extrinsic components of motivation. For the statistical analysis was performed Mann Whitney tests for comparison of groups and chi-square test for the association of variables. After the data were interpreted, the results indicated that students in rural areas were more motivated than students in the urban perimeter, especially when the comparison was made between female students and at more advanced levels of schooling. In line with these data, when a test of association between the type of school and the motivational classification was carried out, it was found that the most of rural schoolchildren were classified as having a high motivation level. In short, from this study, we verified that the environment where the students study or reside can influence sport motivation. Although it is not clear why rural schoolchildren were more motivated, we believe that in the rural context there is a limitation on the diversity of activities, which contributes to greater attention to sport practices. As in the city the youth has at its disposal a wider range of social coexistence, it is probably that the sport will be divided with other occupations, which contributes to the reduction of the sports motivation when compared to rural students.(AU)

ABSTRACT Objective: to understand the health-disease-care process of the older adult living in rural areas. Method: a qualitative research study from the perspective of Madeleine Leininger's Theory of Diversity and Universality of Cultural Care, carried out with the participation of 19 older adults living in the rural area of a municipality in southern Brazil. Data collection took place in July and August 2018 by means of semi-structured interviews and simple observation. And data analysis was performed following Laurence Bardin's Content Analysis proposal. Results: the conceptions of health and disease of the older adult living in rural areas are mainly related to the ability and inability to perform the activities of daily living and work, especially with the land and animals. We highlight the care practices of older adults living in rural areas with regard to the use of medications, food consumption and the practice of physical exercise. In addition, the notion about their health condition and the capacity for self-management and adaptation to the challenges of the health-disease-care process. Conclusion: the health-disease-care process of older adults living in rural areas is influenced by social and cultural factors of the context in which they are inserted. This suggests the planning, implementation, development, evaluation and (re)formulation of health policies, programs and actions focused on providing culturally congruent care, which encompasses more than the singularities of the rural area, in the sense of dichotomy in relation to the urban area.


RESUMEN Objetivo: comprender el proceso de salud-enfermedad-atención de ancianos residentes en zonas rurales. Método: investigación cualitativa desde la perspectiva de la Teoría de la Diversidad y Universalidad de la Atención Cultural de Madeleine Leininger, realizada con la participación de 19 ancianos en la zona rural de un municipio de la Región Sur de Brasil. Los datos se recolectaron en los meses de julio y agosto de 2018 por medio de entrevistas semiestructuradas y observación simple. El análisis de los datos se efectuó mediante la propuesta de Análisis de Contenido de Laurence Bardin. Resultados: las concepciones de salud y enfermedad de los ancianos residentes en zonas rurales están principalmente relacionadas a la capacidad e incapacidad de realizar las actividades de la vida diaria y del trabajo, especialmente, con la tierra y los animales. Las prácticas de atención a ancianos residentes en zonas rurales se destacan con respecto al uso de medicamentos, al consumo de alimentos y a la práctica de ejercicio físico; además de la noción sobre su estado de salud y de la capacidad de autogestión y adaptación frente a los desafíos del proceso de salud-enfermedad-atención. Conclusión: el proceso de salud-enfermedad-atención para los ancianos residentes en zonas rurales se ve influenciado por factores sociales y culturales del contexto en el que se encuentran. Eso sugiere planificación, implementación, desarrollo, evaluación y (re)formulación de políticas, programas y acciones de salud enfocadas en proporcionar atención culturalmente congruente, que comprenda no solamente las singularidades de la zona rural, en el sentido de la dicotomía en relación con el área urbana.

ABSTRACT

Objective:To provide information for further development of chronic disease management, by studying the allocation of resources required to provide services in a county in Shanxi province as required by to the pathway of China-Gates Rural Primary Health Care Service project.Methods:A questionnaire survey was made to evaluate the deployment of manpower, equipment, and drugs required by levels of county, township and village medical institutions in the health management services of hypertension and diabetes in the rural areas of Yangqu county, Shanxi province from July to August 2019.Results:A total of one county hospital, 10 township health centers and 101 village clinics participated in the investigation. In terms of service personnel participating in the project, 9 township-level medical institutions were manned with personnel who could provide diagnosis, intervention planning, and follow-up visits, and only one of them had annual evaluators; village-level medical institutions basically didn′t have diagnosis, intervention planning, and annual evaluation staff. In terms of technical equipments, there was a general lack of diabetes assessment equipment at county, township and village level medical institutions. The mandatory equipments for diabetes assessment was equipped for only 80.0%, 79.0%, and 37.8% of the three levels of institutions, respectively. Village clinics lack hypertension assessment equipment and had no diabetes diagnosis equipment at all.Conclusions:The chronic disease management personnel, equipment, and drug supply in the rural areas of a county in Shanxi province are unbalanced among counties, townships, and villages. The quality of chronic disease management services should be improved through effective and rational use of resources.

Objective:To evaluate the functional status and assistive facilities requirements for the elderly with disabilities in rural areas of Liaoning Province, and to provide reference for the configuration of assistive facilities and the construction of further rehabilitation management mode for the elderly with disabilities.Methods:A total of 929 disabled rural elderly people in 15 deep poverty-stricken areas in Liaoning Province from 2019 to 2020 were selected to evaluate the functional status and needs of assistive devices by using general information questionnaire, disability status questionnaire and assistive device evaluation form for disabled elderly people.Results:There were 149 cases (16.0%) with grade 1 disability, 465 cases (50.1%) with grade 2 disability, 178 cases (19.2%) with grade 3 disability and 137 cases (14.7%) with grade 4 disability. The elderly with disabilities mainly had difficulties in long-distance walking, bathing, toilet use, defecation control, short distance walking, squatting, and transfer. The proportion of disabled elderly was 89.8% (834/929), 84.9% (789/929), 82.6% (767/929), 79.9% (742/929), 76.6% (712/929), 76.1% (707/929) and 60.5% (562/929), respectively. With the increase of disability severity, bathing, short walking, metastasis, shopping and preparing meals accounted for the greater proportion of the elderly with disabilities, and the differences were statistically significant ( P< 0.01). The elderly with disabilities had the greatest demand for wheelchairs, followed by mattresses for preventing stress injuries and four-legged walking sticks, with a demand rate of 56.3% (523/929),9.4% (87/929) and 7.0% (65/929), respectively. The elderly with different levels of disability had different needs on ordinary wheelchair, pressured-injury mattress, four-legged walking stick, stool chair, wheelchair with toilet seat and common walking stick, and the needs had statistically significant differences ( P<0.01). Conclusions:The elderly with disabilities in rural areas of Liaoning Province are mainly moderate to severe, mainly physical disabilities, and have great obstacles in self-care and daily living activities, with the highest demand for ordinary wheelchairs, anti-stress injury mattresses and four-legged canes. In the future, further intervention measures and rehabilitation management models should be developed according to the assessment results of the functions and assistive devices of the elderly with disabilities in rural areas, so as to meet the nursing and rehabilitation needs of the elderly with disabilities and improve their life quality.

Methods@#A cluster random sampling method was used to select left-behind students in Grade 3 to 6 in 5 primary schools in the southern rural area of Ningxia Hui Autonomous Region. Their general information were collected, their social anxiety and loneliness were evaluated by the Child Social Anxiety Scale ( SASC ) and Children's Loneliness Scale ( CLS ). The multivariate linear regression model was used to analyze the influencing factors for the social anxiety of left-behind children.

ABSTRACT Objective: To determine the factors that influence physical activity (PA) participation among secondary school learners in the Hlanganani rural area of Limpopo Province, South Africa. Methods: A quantitative approach was used to examine factors that influence PA involvement among 151 students (mean age: 18.14 ± 1.81 years) attending three rural public secondary schools in the Hlanganani rural area of Limpopo Province, South Africa, who volunteered to participate in the study. Data were collected using validated structured questionnaires. Results: Overall, the results indicated that preferring to do other things with their time, exercise is too hard, unsafe environment, and the lack of facilities and time to exercise were cited as major factors that deterred the students from participating in PA. Conclusion: The findings of this study provide practical implications for promoting students' participation in PA in rural schools. It is recommended that schools should be provided with sport facilities that are proximal and safe for students. Additionally, PA programmes should be promoted through campaigns that would motivate more students to participate in view of its potential health benefits.
